This is the 2x teleconverter for manual focus Nikkor lenses.
This one is in good condition except that there is a light mark behind the front glass element that extends from one egds of the glass to the other. It is very light and I did not notice it without a flashlight but it may soften your images slightly when shooting in harsh conditions. Otherwise the glass is clean with no haze or fungus. The body shows moderate wear.
